Our goal at Dental Innovations is to help you reach and maintain optimum oral health. As the first step towards your continued care, please download and fill out our patient forms entirely and bring them to your first appointment. The more we understand your current health history, the better our dentists can care for you.

New Patient Forms

Our patient forms are available online for your convenience. If you are a new or existing patient, you can download, print, and fill out our forms ahead of your appointment. We're happy to help answer any questions you may have.

insurance

At your first appointment, please provide your insurance information, forms, policy holder’s card, and/or group number to expedite insurance processing. Helping you submit your insurance claims is a courtesy we offer all of our patients.

Financial Policy

For all treatment, we'll ask for payment of the portion of fees not covered by insurance at the time of your procedure. Please download our complete Financial Policy to learn more, and contact us if you have questions about payment options.

We also accept CareCredit, a dental and health care financing company that helps make dental costs more affordable, with low-interest rates and monthly payments.

Please contact us to schedule your first appointment. As a full-service dental office, we'll discuss your oral health, evaluate any issues you're currently having, and develop a personalized treatment plan that best fits your needs and schedule.

cancellation policy

We know there are times and situations when you cannot keep your appointment. If this happens, please realize that we need 48 hours notice so we may offer this time for another patient. Cancellations within 48 hours may be subject to a fee.
